You’ve decided you’re ready. Or — more likely — you’ve decided you’re almost ready, and the last thing standing between you and booking the appointment is not knowing exactly what’s going to happen once you walk through the door.
Totally fair. Aesthetic treatment, especially the first one, is a leap. The internet is full of horror stories, slick before-and-afters with no context, and influencers who make it look like a glamorous ten-minute touch-up. None of that is what a thoughtful first lip filler appointment actually looks like.
Here’s the real version — minute by minute, hour by hour, from the time you book to the time your lips fully settle.
Before You Even Walk In
A good first appointment starts with a good first conversation. When you book online or call (828) 230-2016, we’ll ask a few preliminary questions: what you’re considering, whether you’ve had any injectables before, any relevant medical history, and timing.
A few preparation tips for the days leading up to your appointment:
1 week before: Avoid any new skincare actives. Don’t try a new retinoid or chemical exfoliant right before your appointment.
3–5 days before: If your provider approves, start arnica and/or bromelain supplements to reduce bruising risk.
3 days before: If you can avoid them, skip aspirin, ibuprofen, and naproxen. Acetaminophen is fine. Skip alcohol if possible.
24 hours before: Stay well hydrated. Skip alcohol entirely.
Day of: Eat a normal meal. Don’t come in fasted. Bring water. Wear comfortable clothes. Don’t wear lipstick or lip products to the appointment.

Walking In: The First 15 Minutes
When you arrive at our West Asheville location, you’ll be greeted, offered water, and brought back to a treatment room within a few minutes of your appointment time. Our space is intentionally calm — no fluorescent buzzing, no hard sales pitches, no overwhelming retail displays.
You’ll be asked to fill out (or update) a few intake forms covering medical history, medications, supplements, and any previous aesthetic treatments. Be honest and complete here — even things that don’t seem relevant (like a recent dental procedure or a new prescription) can affect treatment planning.
The Consultation: 15–25 Minutes
This is the most important part of your first appointment, and it’s the part that varies most dramatically between providers.
At our practice, the consultation comes first, and the syringe doesn’t come out until we’ve actually agreed on a plan. Here’s what the conversation covers:
- Your goals. What’s bothering you? What do you want changed? What do you specifically not want? This is where we’ll ask you to use your own words rather than show us a celebrity reference photo. (Reference photos are welcome — they’re useful for communication — but the goal is enhancing your face, not borrowing someone else’s.)
- Your facial assessment. We’ll look at your lips in context with the rest of your face: your chin, your midface, your nose proportions, your smile, your symmetry, your skin quality, and how your lips move dynamically when you talk and smile.
- The recommendation. We’ll explain what we’d suggest and why. This might be straightforward filler. It might be a lip flip instead. It might be a combination. It might be PRF if your goal is more about skin quality than volume. Sometimes — and we mean this — it’s “you don’t actually need anything yet.”
- The product choice. If we’re using filler, we’ll explain which filler we’re recommending and why. Most often that’s Teosyal RHA or Restylane, depending on your goals.
- The dose. We start conservative. For first-time clients, that’s usually ½ to 1 syringe. We talk through the reasoning in our how much lip filler you actually need guide.
- The cost and the consent. Transparent pricing. No surprise add-ons. You sign informed consent only after you’ve heard the full plan and asked any questions you have.
- Your questions. This is your time. Ask anything. There’s no such thing as a stupid question, and we’d much rather you ask now than wonder later.
Numbing: 15–20 Minutes
Once we’ve agreed on the plan, we apply topical numbing cream to your lips and let it work for 15–20 minutes. You can scroll your phone, read, or just breathe. Many of our HA fillers (including all of the RHA line) also contain lidocaine in the syringe, which adds additional comfort once injection begins.
The Injection Process: 10–20 Minutes
Here’s what actually happens:
You’ll lie back in a comfortable position. Your lips will be cleaned with antiseptic. We’ll mark your treatment plan if needed.
Then we inject — slow, careful, in small amounts at planned points. You’ll feel some pressure, occasional small pinches, and possibly some warmth. Most clients describe it as 3–5 out of 10 on a discomfort scale, with the numbing and lidocaine taking the edge off significantly.
We pause throughout to assess. Massage gently. Make sure both sides are balancing as we go. Sometimes we sit you up to look in a mirror partway through to check shape from your normal viewing angle.
The whole injection process usually takes 10–20 minutes for a single syringe. Faster than most people expect.
Immediately After: 5–15 Minutes
Once we’re done, we’ll have you sit up, look in a mirror, and review the immediate result. Your lips will be swollen — that’s normal — but you’ll be able to see the shape and the direction of the change.
We’ll provide:
- An ice pack
- A printed aftercare sheet
- Recommendations for any specific products that support healing (we carry medical-grade options if you’d like to upgrade)
- A scheduled or open-invitation 2-week follow-up
You’ll be on your way within 60–90 minutes of arriving.
The First 24 Hours
Expect:
- Continued swelling (peaks at 24 hours)
- Possible tingling or numbness as the lidocaine wears off
- Some tenderness when eating or drinking
- Possible small bruises at injection points
- Lips that look bigger than your final result will be

Days 2–7
Swelling resolves day by day. Bruises (if any) move through their color stages and start fading. By day 5–7, your lips will look much more like the final result, though not quite settled yet. Most social activities are completely back to normal by day 3.
The 2-Week Mark
This is when your filler has fully integrated and you can see your true result. Your lips will:
- Have settled into their final shape
- Move naturally when you smile or speak
- Feel soft and natural to the touch
- Reflect the goals you set in your consultation
If anything doesn’t feel right at the 2-week mark, this is when we want to see you again. We do complimentary 2-week follow-ups because catching anything early is much easier than addressing it later. For broader recovery context, see the aesthetic recovery hub.
What If I Don’t Love It?
Honest answer: this is rare when you start conservative with the right provider, but it happens. A few options:
The result is too subtle. Add more product at your follow-up. (We started conservative on purpose precisely so this would be an option.)
There’s a small lump or asymmetry. Often resolved with massage at the 2-week visit; sometimes addressed with a tiny additional placement.
You genuinely don’t like the result and want it removed. HA filler is reversible with hyaluronidase. We can dissolve it. See our dissolving lip filler guide for details.
This reversibility is one of the reasons HA fillers are a good first injectable. You’re not making a permanent decision.
